California Healthy Kids Survey

The California Healthy Kids Survey will be administered annually. Student participation in the survey is voluntary, but highly encouraged.

Permission slips with additional information are sent out to families. Grade levels participating in the survey include Kindergarten through 8th grade. Samples of the student surveys are linked below. If you have additional questions about the California Healthy Kids Survey please contact your child's school.

To participate, students in 3rd through 6th grade require a signed consent form from a parent/guardian. For grades Kindergarten through 2nd grade and 7th through 8th grade, students will participate unless a parent/guardian opts them out.

California Healthy Kids Survey is administered every year for students 3rd through 8th grade statewide. In Wiseburn, the survey for Kindergarten through 2nd grade students is a modified version of the California Healthy Kids Survey, intended to collect helpful data on our school experience for our youngest learners.
