Caregiver Authorization Form
This form is for families and caregivers when a student is living in the home of an adult who is not the student’s parent or legal guardian due to temporary changes in family or housing circumstances. The Caregiver Authorization Affidavit allows a qualified caregiver to enroll a student in school and authorize school-related medical care, as permitted under California Education Code section 48204 and California Family Code sections 6550–6552.
